Forms to Assist in Making Referrals to T.E.A.M.

Making the decision to formally refer an employee to T.E.A.M. can be a daunting responsibility. To support you in this undertaking, T.E.A.M. has several forms that will assist you with the referral process. These forms are provided in a format that will allow you to print off a copy that retains its presentation integrity. In order to access these forms it is necessary that you utilize Adobe Acrobat Reader. Formal Referral Paperwork
This three page document encompasses the paperwork that you will give the employee to formally refer him or her to T.E.A.M., along with the Client Information Release Authorization that will allow T.E.A.M. to provide you feedback about the employee’s involvement in counseling. The employee’s signature is required on both parts of this document to activate the referral process.

Post-Formal Referral Agreement
Upon conclusion of the employee’s involvement in the formal referral process, T.E.A.M. recommends that the employee be asked to sign a statement agreeing that he or she has received the requisite assistance through the EAP.

Fitness For Duty Checklist
At times it may be difficult to determine what factors should be present in order to question an employee’s fitness for duty. This two page form will assist you in objectively identifying characteristic appearances and behaviors that warrant questioning an employee’s duty fitness.

List of Red Flags For Potentially Violent Employees
This FBI list of characteristics and pre-incident behaviors of potentially violent employees can guide you in recognizing when the referral of an employee to T.E.A.M. is a prudent step to take to ensure the safety of your organization.
