T.E.A.M. is always interested in hearing from you, our Affiliate
Providers who are valuable members of our EAP team. You have several
mechanisms through which you can contact us.
E-Mailing T.E.A.M.
- General Questions
If you have general questions about your affiliation with T.E.A.M.
or a T.E.A.M. client, click here.
If you are referencing a specific case, please include the T.E.A.M.
case number and client's name. Please note: If your situation
requires an urgent response, please contact T.E.A.M. by phone
rather than e-mail.
- Suggestions/Complaints/Grievances
If you have a suggestion, complaint or grievance about working

- Authorization for Additional Client Sessions
At this time authorization for additional sessions with a T.E.A.M.
client must be done by phone during regular business hours (9:00
a.m. - 5:00 p.m. Eastern U.S. time). Please ask to speak to the
Client Services Supervisor.
- General Questions
If you have general questions about your partnership with T.E.A.M.
and are more comfortable speaking with T.E.A.M. by phone than
you are by e-mailing, feel free to call us during regular business
hours (9:00 a.m. - 5:00 p.m. Eastern U.S. time) and ask to speak
with a member of the Affiliate Services Department.
- Emergency Situations
For any emergency situation regarding a T.E.A.M. client, please
contact (248) 680-4611. Describe the emergency that needs immediate
attention and your call will be directed to the appropriate party.
