As a valued T.E.A.M. Affiliate Provider we want to keep all of the information about your organization as up-to-date as possible. To help you keep T.E.A.M. apprised of any such changes, we have developed several easy-to-use forms that you can e-mail directly to T.E.A.M.

Organization
To update information about your organization, including address, satellite offices, phone number, contact person, staff or e-mail address, click here.

Billing
To update information about your billing address, billing phone number, billing contact person, or billing e-mail address, click here.

Credential Updates
To check to see if your clinical credentials or proof of liability insurance need to be updated, click here.

Individual Application
In addition to the organization application, each clinician from the organization who would like to be credentialed as a T.E.A.M. provider must complete an individual application.
