If you are interested in applying to become a T.E.A.M. Affiliate Provider, we recommend that you review the detailed list of T.E.A.M.'s Affiliate Provider qualification requirements. Prospective Affiliate Providers must complete a written application as well as provide T.E.A.M. with copies of current state licensure or certification, highest degree diploma, current resume, proof of liability insurance (minimum: $1 million per claim/$1 million aggregate), and a detailed malpractice history.
