who can serve as T.E.A.M. Affiliate Providers. Some of the many
benefits for a provider that affiliates with T.E.A.M. include:
- Minimal paperwork requirements
- No invoicing required
- Prompt reimbursement for services rendered
- Case consultation from T.E.A.M. available
- Authorization for additional EAP sessions
- Clinician can continue to work with EAP clients who need long
term treatment when clinically appropriate
If you are interested in applying to become a T.E.A.M. Affiliate
Provider, we recommend that you review the detailed list of T.E.A.M.'s
Affiliate Provider qualification
requirements. Prospective Affiliate Providers must complete
a written application as well as provide T.E.A.M. with copies of
current state licensure or certification, highest degree diploma,
current resume, proof of liability insurance (minimum: $1 million
per claim/$1 million aggregate), and a detailed malpractice history.
Approved providers must also forward updated copies of licensure/certification
and liability insurance to T.E.A.M. on an annual basis. To complete
